No. Preventiva Medical Plan is a discount medical plan (a Discount Medical Plan Organization under Chapter 636, Florida Statutes). It is not insurance and does not replace an insurance policy. You pay the provider directly, at a reduced rate that we have negotiated for members.
No. Preventiva works on its own, with or without insurance. Many members have no insurance at all; others use Preventiva for services their policy does not include.
No, none of them. There is no eligibility review: anyone can become a member, whatever their immigration status, income or medical history.
No. There is no medical exam, no age limit and no exclusion for pre-existing conditions. It is a membership, not a policy, so nobody is turned away.

Individual is for 1 person. Couple is for 2 (primary member plus spouse). Family is for up to 5: primary member, spouse and 3 dependents, or primary member and 4 dependents.
Yes. You can move to a larger membership at any time and the new members get their digital cards once the change is applied.
Individual is $35 a month, Couple $40 and Family $45. All three have a one-time $30 activation fee, charged only when you enroll.
It depends on the provider, the location and the service received. Preventiva does not guarantee a specific discount amount, and provider participation may change without notice. Ask for the rate at the provider before your appointment.
No. There are no deductibles, no copays and no surprise bills. You pay your monthly membership and the reduced rate directly to the provider at the time of service.
The membership renews monthly and is charged automatically to the card you register when you enroll. You receive a receipt by email for every payment.
Right away. Enrollment takes a few minutes and is entirely online; as soon as the payment goes through, your digital card is issued.
