Preventiva Medical Plan LLC does not make payments directly to providers for medical services. The member is obligated to pay for all health care services received but may receive discounts from participating providers pursuant to the membership plan.
Preventiva Medical Plan LLC is not responsible for the quality of services rendered by participating providers. Providers are solely responsible for professional services provided to members.
